Planned mode of birth and longer-term maternal outcomes.

DESCRIPTION OF PROJECT

More than two in every five births in the UK now occur by caesarean section. Clinical guidelines have increasingly advocated for birth mode choice and engaging women in evidence-based discussions about the benefits and risks of planned caesarean compared with planned vaginal birth to support informed decision-making about childbirth. However, as highlighted by the National Institute for Health and Care Excellence, evidence on the longer-term benefits and risks of planned caesarean compared with planned vaginal birth in singleton term pregnancies remains limited.

 

This DPhil project aims to assess the effects of planned caesarean versus planned vaginal birth on longer-term maternal outcomes. This could include examining longer-term health outcomes such postnatal mental health problems, sexual dysfunction and pelvic floor dysfunction, as well as outcomes in subsequent pregnancies such as preterm birth.

 

The project will use large-scale linked routinely collected (real world) health data, combined with the latest methods for improving the quality and transparency of evidence derived from such data, such as target trial emulation. It will also use data from a national maternity survey in England.

 

The research will generate evidence to support women and clinicians in making informed shared decisions about birth options. Findings may also help improve women’s pregnancy and birth experiences, identify potential targets for intervention and contribute to the optimisation of maternal outcomes.

 

Candidates should have a strong quantitative background and experience in statistics, epidemiology or a related discipline.

  

TRAINING OPPORTUNITIES

The student will receive training in the design, conduct, analysis and reporting of epidemiological research using large-scale routinely collected health data. They will also receive training in the analysis of survey data.

The project will also provide opportunities to gain experience in scientific writing and presentation, patient and public involvement, and working with interdisciplinary collaborators. 

Training will be provided through supervision, working with team members and attending short courses, as appropriate.
